How to design it with Photoshop ?  

  Open new document 850 x 625 pixels with a violet background.

1. Use Custom Shape Tool ( press U in the keyboard) in the tool box ,and click the shape button  and choose the shape of the heart ,then go to style  button and choose red or green or blue gel colors, and draw with your mouth.
2. Use the add layer style button under the panels to apply effects like stroke bevel ..... and others.
3. Choose frame from the shape ,choose the style and draw the frame around the borders   .
4. Save your work.
